WAC 246-810-031.pdf Download WAC 246-810-031.pdf  Open the WAC 246.810.031 either through the Washington State Legislation page https://app.leg.wa.gov/wac/default.aspx?cite=246-810-031Links to an external site. or from the pdf attached.  This is the real, legal documentation you must follow with a two-year degree credential.   Follow each and every guideline to make your own disclosure document.   I don't mind if you make up information... like your PhD, etc... for the sake of the assignment, but you must include the information in the WAC as a two year credentialed counselor.  (Any documentation found on the web may not include all that is required for a two year person under this WAC.  A person with a Masters or Phd might not have the same requirements and you will lose points if you don't include everything.) Special note: under the list notice that item (h) The limits of confidentiality under RCW 18.19.180Links to an external site.. will require you to open that RCW and use it.  Also you must include the list of unprofessional conduct RCW 18.130.180Links to an external site. , and the purpose of the Washington state credentialing act, also found on the WAC.  I will subtract 10 points for each item missing from the list.  I have included two examples... the student example is a .tif and doesn't download as a word document.  The other document above is a real disclosure used by professions that are LICENSED.  This is only an example.... do not follow as you will be missing valuable pieces required by Certified ADVISORS with a two year degree.  (Also, any examples you find on the web are likely to be for licensed therapists as well... they do not work for this assignment... you will lose points.  Follow the WAC!!!!  I don't mind if you make up your degree and experience... but you must have all the requirements for a person with a 2 year degree as a Certified Advisor.)
